Contact & Referrals
The state of one's health influences all aspects of living.
End-stage heart disease effects not only physical feelings, but mental and emotional statesthose of the patient, their family and other loved ones too.
Not only complex surgical and medical issues must be faced, but psychological, financial and social ones too.
To help our patients and their families steer the best course to recovery, NewYork-Presbyterian/Columbia Cardiac Transplantation Program is based on a multi-disciplinary approach to the healing process.
This means that we combine the services of a wide array of professionals to provide care during the entire transplantation process.
Typically, patients are referred for evaluation in our program by their cardiologist, primary care physician, or family doctor.
However, you can contact us directly for an evaluation.
The purpose of the evaluation is to determine if transplantation is the right choice for each individual's particular medical, emotional and social support situation.
Because of the stature and scope of our service, we receive patient referrals from around the country, and around the world.
If accepted into the program, these patients will need to relocate to the New York metro area during the latter stages of the waiting period and for the three months following surgery.
